fix: reset MSAL instance on each login attempt to clear corrupted state

After a failed device-code flow, the MSAL PublicClientApplication retains
error state that causes subsequent startDeviceCodeFlow() calls to fail
immediately. Reset the PCA instance at the start of each new login attempt
so Try Again always produces a fresh, working device code.
