Release v0.2.3 — zombie-client fix + per-realm token history

Bundles everything since v0.2.2: SDK patches 5+6 (discarded clients no
longer retry forever after a 401; real ws error surfaced), per-realm
token recall with keychain-encrypted host history, cross-realm token
leak fix (UI v26-v35). Also repins INSTALL.md (was v0.2.1) and syncs
package-lock.
