Fixes

  • External review bounces no longer incorrectly refuse to requeue because the card's own PR-reconcile task is counted as "still working the card."
  • A Vera FAIL now correctly triggers a fix round instead of remaining in a deferred state for extended periods.
  • API and tool callers retain the same refusal behavior for requeue_refusal and are unaffected by this change.
