Fix Windows transcript discovery

Root cause: Cursor on Windows stores transcripts as flat .txt files directly in agent-transcripts/ (e.g. <uuid>.txt), not in subdirectories with .jsonl extension like macOS/Linux (<uuid>/<uuid>.jsonl).

  • New resolveTranscriptFile and listTranscriptEntries helpers that transparently handle both layouts
  • All transcript functions updated: listProjects, listChats, getChat, getChatFileSize
  • Projects with chats now correctly appear on Windows
