• Restore updateIfStalerThan(durationMs) for serverless / lambda use (qfg-q612). Inlines a
  conditional refresh when the in-memory envelope is older than durationMs, otherwise no-ops.
  Intended to be called at the top of each request in environments where background timers (SSE,
  fallback poll) are paused while the container is frozen. Returns the in-flight Promise<void>
  when a fetch is started, undefined when still fresh / already refreshing / running from datadir
  or datafile / closed. Concurrent calls coalesce onto the same in-flight promise. Method existed in
  the original Reforge sdk-node and was lost during the Quonfig rewrite.
