Found via real-world use: --detect-equations --recover-clean-equations detects equations correctly but skips splicing them back in whenever the page was native-trusted, logging "no PageOutput; skipping" — which is exactly the case the feature exists to fix.
Mac host; CE digits skipped; no adapter.
