Breaking Changes

• dekk agents and dekk worktree now require the app name. Use dekk myapp agents init instead of dekk agents init. This ensures worktree-safe routing — dekk resolves the correct .dekk.toml from the app name before running any project tool.

New

• src/dekk/tools/ package — extensible tool registry. Each built-in tool (agents, worktree) lives in its own folder. Downstream CLIs can register additional tools by adding entries to REGISTRY.
• Constants for all tool names — AGENTS, WORKTREE, CLI_NAME, NAMES defined once in dekk.tools, no hardcoded strings.
• Helpful error messages — running dekk agents init now tells you the correct command: Use 'dekk myapp agents init'.

Docs

• README, docs/agents.md, and docs/worktree.md updated to reflect dekk myapp agents/worktree pattern.
• Both patterns documented: dekk myapp (recommended, worktree-safe) and Typer inheritance (alternative for downstream CLIs).
