Relay now returns a RelayResult with CloseReason (normal/limit/network)
and Direction (client/backend) instead of a bare error. This surfaces
which side closed the connection and why in both the "connection closed"
INFO log and the OTel close span, enabling Datadog filtering on
@close_reason and @initiated_by to diagnose premature closures.

Limit errors are now WARN-level; error sentinels are exported for
external callers.
