Prepares effect-qb and effect-db 0.22.0 after #17.

Why

The querybuilder capability work includes intentional public API breaks, so it advances the pre-1.0 minor version.

Changes

• bumps both published packages and the effect-db CLI to 0.22.0
• updates workspace lockfile versions
• adds generated release notes since v0.21.0
• tracks the release in effect-qb-8m9

Validation

bun run test:types
bun test
bun run build
bun run test:pack

Local result: 550 tests passed; type checks, package builds, and packed consumer smoke passed.

CI will repeat package verification on Bun and Node 22/24 plus live PostgreSQL, MySQL, and SQLite integration.

Publish

The tag has not been pushed. After this commit lands on main, v0.22.0 will point at the integrated release commit and trigger trusted npm publishing plus the GitHub Release.

Rollback

Revert the release commit before tagging. Once published, ship a follow-up version rather than moving the accepted tag.
