The extension has sat at 0.8.0 while 30 commits landed in the shared UI the
webviews compile at build time, so graph, health and refactoring improvements
were written but never delivered. This rebuild ships them.

Raises MIN_SERVER_VERSION to 0.45.0. The refactoring views read the unified
recommendation contract introduced in 0.45.0, so an older server cannot answer
them. Not 0.46.0: nothing in that range added a contract the webviews depend
on, and a needless floor would block working 0.45 servers.
