CLAUDE.md still described the repository as holding "the scaffold only"
and pointed a reader at the five founding ADR beads plus the operator
gate bead rather than at the records those beads produced. Both claims
have been false since the vault, envelope, Kdf and provider stack
landed.

Rewrites the two paragraphs against the shipped surface, and routes the
"read before writing" section at docs/adr/README.md and each record's
own status line - which is where an amendment's separate status lives,
since an amendment can sit proposed under an accepted record. Adds the
rule that acceptance is the operator's reading, which the old text
carried implicitly through the gate bead.

Also drops the "moduledoc-only scaffold" clause from the sentence
describing the coveralls.json deviation; the deviation is about files
with no relevant lines, which outlives the scaffold.

No Elixir code changes, so no gate run and no changelog fragment
(changelog.d/README excludes documentation and agent tooling).
