Bumps @version to 0.4.1 and assembles the two unreleased
fragments into a 0.4.1 section, dated today, per the
changelog.d README. Both are Fixed entries and the fragments
are deleted in this same commit. Every bullet carries over
verbatim; nothing above or below the new section moves. No
fragment carried a Breaking heading, so the section opens at
Fixed.

The README install snippet is left at the exact-minor pin it
was deliberately moved to after the 0.4.0 prep, which is the
form the pre-1.0 banner recommends and which already admits
this patch release. The 0.4.0 prep's exact-version pin is
superseded by that later decision.

The tag and the Hex publish are not here: this commit stops
at the bump and the promotion.
