The operator's acceptance reading of 2026-09-13 covers every amendment
section these four records carried at proposed: the 2026-08-27 sets on
ADR-0001, ADR-0002, ADR-0003 and ADR-0004, the 2026-08-28 amendments on
ADR-0003 (the salt is an extract) and ADR-0004 (Q2 is answered), and
ADR-0003's Amendment C (the per-index Argon2id salt).

Each status line now reads accepted with the date; the headings carry
both dates; the ADR index and the README table say "with amendments"
for 0003 and 0004 and drop the awaiting-a-reading paragraphs. The one
status claim in lib (checkpoint.ex's "status: proposed" on ADR-0002
amendment 6) is updated; section-name citations of the form "proposed
amendment N" are names, not claims, and stay.
