ADR-0002 decision 10 gains G15, core.map's row, read off the module
sb-kqno shipped in PR 281 rather than written ahead of it: the four
fields with their labels, the fixed done/error outcomes, the single
<invoke> of the constant fan-out type with chart on src and as a
param, the collect <assign> on the success transition, and the
statement that this row validates nothing about N because the bound
is the fan-out runtime's. ADR-0009 decision 11 assigned the row to
the implementing bead; that bead touched no docs/adr file, so it is
written here. The ordinal moved: core.await took the sixteenth, so
this is the seventeenth, and a dated Note beside G11 records that
the table and the palette now both read seventeen.

ADR-0009 decision 4 gains a dated Note beneath its declaration
table: the shipped surface spells assign_to and aggregate as collect
and on - the second by the operator's RQ-031-4 amendment, which has
the scheduler read the policy off the on param verbatim - and
item_as, index_as, max_concurrency and params are deferred to a
later bead rather than dropped, since a drop is a record decision
this Note does not make.

Amend-by-addition throughout: zero removed lines in docs/adr.
