Hyper-V Clean Room v0.3.2

This source-only compatibility patch accepts optional object-shaped MCP _meta on tools/call while preserving closed outer parameters, the exact 20-tool API, schema-v1/v2 behavior, Plan/Apply boundaries, and fail-closed safety semantics.

The selected app-server acceptance path was validated with a child-local test-only mock adapter: inspect_host proved changed=false and the mandatory TEST_ONLY_MOCK_ADAPTER warning before list_vms was allowed. Both mock calls passed, and every real-operation counter was zero.

Safety disclosure: an earlier source-iteration harness set mock variables only on the app-server parent and made one unintended production read-only inspect_host call. It returned ok=true, changed=false, caused no mutation, and stopped before list_vms; it is not release acceptance evidence. The external production typed read-only smoke remains notPerformed, and no real Hyper-V, VM, checkpoint, credential, guest, package, UI, network, evidence, or manual-attestation operation was performed in this gate.
