Bug fixes to new forms

This release addresses three bugs with the new forms:

• When entering data for a Resource Request form, Item #2 would be skipped.
• When entering data for a Shelter form, if the shelter type was not Congregate, validation warnings would be printed for each shelter type until one was selected. Now the warning will only be printed after all five shelter type fields have been visited and none of them were checked.
• When entering data for a Windshield Survey form, it now properly enforces that one of the item types must be selected.
