Refactor image decoding and rendering modules

Summary

• Split image handling into focused decode, render, terminal, orientation, and external modules
• Keep the image pipeline behavior intact while reducing src/image/mod.rs complexity
• Update repo guidance in AGENTS.md to reflect the new structure
