Add setup instructions and improve settings page UX

- Add "How It Works" 4-step guide shown before connecting
- Add expandable "Where do I find the embed code?" help in Form Generator
- Improve post-creation message with clear next steps
- Add CSS for step indicators and help toggle
- Bump version to 1.0.2
