• Internal: orchestrate.rs (1300+ lines containing every CLI command in
  one file) is now split into core/commands/{lifecycle, scaffold, registry_commands, shell_integration}.rs, grouped along the command
  structure already implicit in the CLI. Pure refactor, verified with the
  full test suite plus a live smoke test across all four command groups
  before release — nothing about how stack behaves changes in this
  version.
