feat(cli): add --strategy flag to customize AI prompt instructions

- Implement 'first-line' and 'range' strategies for the clipboard prompt.
- 'first-line' instructs the AI to read only the first line to bypass read preconditions.
- 'range' instructs the AI to read specific code ranges before modifying.
- Default to 'first-line' as a safe fallback.
