Changes

• Enrich web_search fallback results with visible SERP titles, URLs, and snippets so blocked-site recovery has usable evidence immediately.
• Add extract_links and extract_images to the large read-result lifecycle to avoid repeated oversized link/image tool outputs bloating context.
• Fix Anthropic Haiku calls by omitting output_config.effort unless extended thinking is active.
