Bug-fix release: CLI flag parsing, store races, server hardening.

Fixed

• CLI flag values eaten as task text — mark.mjs add --project foo "fix bug" recorded "foo" instead of "fix bug"; same class of bug in remind.mjs with --at/--in/--every/--project. Both now pick the real positional.
• Lost updates on concurrent writes — the CLI, hooks, and dashboard server could race on the same JSON files and silently drop writes. All store mutations now run under a cross-process lockfile (stale locks are taken over; contention falls back to unlocked after 5s so the plugin never deadlocks). Atomic renames also retry on Windows EPERM/EBUSY.
• Hooks hanging 500ms per prompt — the stdin fallback timer in the capture hook kept the process alive; it no longer holds the event loop.
• Full transcript loaded per prompt — the nudge hook now reads only the last 1MB of the transcript instead of loading entire multi-GB files into memory.
• Dashboard server crashes — oversize request bodies now get a proper 413 instead of hanging the route; store errors on mutations return 500 instead of killing the server; a second server instance on the same port prints "already running" instead of a stack trace.

Tests

Test suite grew from 56 to 73 checks, including regression tests for every fix above.
