🚢 Fixed

  • Worktree cleanup now uses persisted final PR status for completed tasks, avoiding recurring maintenance failures from historical tasks when GitHub PR-state reads fail.
  • Open PR tasks still refresh live PR state, and cleanup failures now include task, PR, and worktree context while the sweep continues to later tasks.
