Fix

Tab focus no longer triggers an infinite reload loop. Redundant tab.focused events (including echoes from pane moves) now no-op when agent and sidebar are already live on the tab, instead of re-docking and re-focusing in a ping-pong cycle.

Also in this patch

• Pane lifecycle recovery — after a Herdr server restart, empty layout panes (shell, review, sidebar) are relaunched once per 15s cooldown.
• lifecycle.sh — pane process helpers extracted from layout.sh for clearer separation from topology/docking logic.
