Add subdomain to fields sent in request logging

Adds a new method, ContextWithSiteAndUserInfo(), which can be used by a spec-implementing service to add a Skuid Site's Id and Subdomain, as well as a User's Id, to a request context, and modifies the logging middleware to include "subdomain", if it is defined, as an additional field in structured request logging.
