v0.24.4 — MCP tool-definition enrichment

• @statewavedev/mcp-server → 0.4.8 — every MCP tool now documents its parameters and behavior in the tool description + JSON-schema annotations (e.g. the repo:owner/name → repo:owner.name subject-shape note; the server rejects slashes in subjects). LLM clients calling the server see richer, self-describing tool schemas. Descriptions / schema annotations only — no behavior change.
