v0.4.1 - Fix Debugger on Marketplace

• fix broken plugin version 0.4, the version uploaded to the marketplace was
  not able to run the debugger
• update LSP implementation for Java, moved to org.eclipse.lsp4j code base
  (same project as before, but now an official Eclipse project)
