`semantic_search` and `hybrid_search` used to return a bare
`{error: <exception_string>}` when called without the optional
embeddings setup. LLMs read this as catastrophic.
Both tools now return `{status, message}` with two distinct status
values:
- `embeddings_not_enabled` — `codegraph.embed.query` import failed.
Message tells the user to `pip install polycodegraph[embed]`.
- `embeddings_not_built` — module imported fine, no index for the
repo. Message tells the user to run `codegraph embed`.
Both messages explicitly note that structural query tools (find_symbol,
callers, callees, blast_radius, dataflow_trace) work without
embeddings, so the LLM doesn't escalate the partial degrade.
