docs: name it 'The Dossier Machine aka Research Entity' - #2

The brand name alone gave a reader no way to connect the repo to the /research-entity command they type or the package name in every manifest. Both halves now appear together in the README title and the Claude and Codex displayName fields. The CHANGELOG line recording the original rename is left alone — it is the historical record of that change, not a live label.
