cli:sparkwing doctor reports ref worktrees left behind by a submission
whose run the store no longer holds. It reports them rather than removing
them, because deregistering a worktree needs git and doctor deliberately does
not reach the orchestrator; a consumer reclaims them as it starts.
Fixed
cli: the foreground --sw-ref worktree is deregistered when its run ends.
Cleanup removed the directory but never pruned, so a git worktree remove
that failed left the origin repository holding a registration that blocked
adding the same path again.
cli:runs submit explains its --sw-index refusal in terms of what the
flag does. It described the index as a path the submitting process holds open;
the binding is an environment variable naming a file sparkwing neither creates
nor can reproduce, which is the reason a detached run cannot honor it.
