What's changed

• Redact every occurrence of each prompt-injection pattern, not just the first match, in stored thoughts, echoed output and history (#99). Thanks to @vishalhabib99 for the clear report and repro.
• Update the Vite+ toolchain to 1.1.0, so tests now run on Vitest 5.
