dsh-cbm v0.2.0

Resolve codebase-memory-mcp from CBM_COMMAND, PATH, or the standard user-local install directory.
Remove the plugin's default CBM_ALLOWED_ROOT restriction so external repository paths remain indexable like Codex.
Keep CBM_ALLOWED_ROOT available as an optional inherited CBM security boundary.
Add runtime resolver tests, TDD evidence, and updated architecture/specification documentation.
