Highlights

• Usage & Spend, rebuilt: explicit cost provenance and coverage everywhere — token mix, an hourly activity heatmap, an All-time range, per-request custom pricing overlays, and opt-in read-only OpenCodex import. Estimates are labeled, partial totals show their coverage, and nothing unpriced masquerades as a real bill.
• Settings window fixed for good: the invisible keepalive-window hack is gone (#3003) and its regression is repaired with a proper retained window controller (#3029) — Settings now opens reliably, in front, every time.
• Crash fixes: the fatal CloudKit sync assertion (#3030) and misleading Codex RPC timeout errors (#3022) are gone.

• Settings: replace the selector-based Settings opener with a retained window controller, fixing silently failing or behind-other-apps Settings opening after the keepalive removal, and repairing localized application-menu commands (#3029). Thanks @Zihao-Qi!
• Codex: classify app-server request timeouts before terminating the subprocess, so timeouts no longer surface as misleading EOF/malformed-response errors (#3022). Thanks @Chipagosfinest!
