Assessments

This release includes a new workflow for recording an assessment for a learner:

Button added to student interface that, when clicked, notifies instructors via Slack.
When coach marks assessment complete, a dialog appears where each learning objective has to be verified and a note must be entered.

The student detail overlay now includes buttons to perform the following actions:

Permanently delete all student records.
Soft delete student records. User account remains but all previous progress through the course is removed (useful for restarts).
Transfer student to a different cohort

Instructors can now view students in each active cohort in the student card list instead of needing to switch to a cohort in the cohort list.
