External-header LSP continuity (0049 §4): a definition jump out of the
working tree keeps its language service.

Fixed

  • gd from a project file into a library/system header (e.g.
    vector.hpp) no longer loses the language server: the jump carries
    the replying server's context as a routing hint, didOpen follows
    through it, and the next gd inside the header answers. Verified
    against real clangd: extensionless vector opens with
    languageId: cpp.
  • Extensionless and ambiguous C/C++ headers (.h) inherit the
    navigation's language instead of falling to plaintext or C.
  • The same external header reached from two projects keeps the first
    navigation's context — no silent compile-context switch.
  • A manually opened file no server covers gets a truthful route ("no
    language context for this file — reach it via gd from a served file,
    or add its root to languages.toml") instead of install advice; the
    install hint is reserved for languages with no server at all.
  • Namespaces never cross: a remote jump context never binds a local
    document, and server death drops both bindings and carried contexts.
