Refactor roles are now unmistakable

• Renames Stage 40 to /speckit.engloop.40-refactor-plan.
• Stage 40 works with the user to read and cite the configured North Star, applicable architecture decisions, Component Pattern, and relevant learnings; it plans architecture conformity and generic/non-vertical extraction into components without changing product code.
• Stage 04 remains /speckit.engloop.04-refactor and is implementation-only: it requires an accepted SPEC/REFACT/repair slice, rereads binding North Star/architecture guidance, and cannot silently re-plan or widen scope.
• Keeps explicit point, bounded, and deep planning profiles with deterministic session binding.

Positive history

• Adds /speckit.engloop.23-happy-minute and global HAPPY<NNN> records.
• Happy Minutes are gratitude-first and lightweight; selected records can later become positive Stage 42 provenance.
