Patch release completing Stage 21 postmortem continuation recovery.

  • Fixes the remaining v1.15.3 existing-gate path: unrelated command-style options no longer trigger Stage 21 scope parsing, and supplying only --incidents or only --postmortem no longer terminates the active chat.
  • Partial relevant scope updates preserve the valid identity-bound gate but create an ignored denial-only marker. PreToolUse denies every tool and Stop accepts no completion until the complete original incident/PM pair passes the existing argument, HEAD, assembly, manifest, and package checks.
  • Plain follow-up text cannot clear suspended authorization; missing values are never inferred from previous prompts; corrupt suspension state denies rather than authorizes.
  • Strict postmortem-learning completion validation and all Stage 22 repair gates remain fail closed.
  • Release documentation now lists all four immutable assets, including the SHA-256 manifest.
  • Exact surface: 26 commands/agents/prompts; .NET SDK 10.0.303; SEK v0.1.3.
  • Whole-product readiness: PASS. 3,322 generated tests, 267 core/direct tests, and 64 overlay tests passed. All six modules exceed 95% line and branch coverage; Tool is 97.99% line / 95.18% branch.
  • Final CI, exact SEK setup, isolated tool install, disposable extension install, agent-surface validation, and clean/coexist overlay transactions: PASS.
