First public release of Codex Provider Manager.

Downloads (self-contained, no .NET required):

• CodexProvider.exe — Windows x64
• CodexProvider — Linux x64 / macOS x64 (from CI artifacts; the two POSIX builds share a filename, see job artifacts for the per-OS file)
