Bug Fixes

• Fix panic on multi-byte characters: Safe UTF-8 truncation using floor_char_boundary — fixes crash on source files containing emojis (🇬🇧, 🇺🇸) at the 2000-byte body truncation boundary
• Skip node_modules and dependency directories: Hardcoded ALWAYS_SKIP_DIRS list as defense-in-depth (node_modules, vendor, pycache, target, build, dist, .next, .nuxt, .cache, etc.)

Impact

• Borough project: 86,561 files → 2,554 files (99.7% reduction)
• Any codebase with emoji in source code no longer crashes during indexing
