v0.2.4 — MCP Tool Adoption Fix

What's Changed

A/B testing revealed Claude Code barely used CodeGraph tools (2/44) despite them being available. This release fixes MCP tool adoption through 4 complementary mechanisms:

CLAUDE.md Template Rewrite

• Decision matrix: "Instead of Grep for X, use codegraph_Y" — teaches when to use each tool
• Anti-patterns: Explicitly says "Don't launch Explore agents, use codegraph_dependencies"
• Compressed tool list (44 tools in 5 lines vs 44 individual lines)

MCP Tool Descriptions Enhanced

• Added "Use instead of X" guidance to 12 key tools
• Example: codegraph_callers now says "Use instead of Grep — 100% precise, no false positives"

Server Instructions

• MCP initialize response now includes prescriptive tool preference guidance
• .mcp.json gets serverInstructions field for client-side reinforcement

Hook Enhancements

• SessionStart: Appends "Use codegraph_* tools — faster than Grep/Glob/Explore"
• PreToolUse: When Grep/Glob detected, suggests CodeGraph alternatives
• PostToolFailure: Suggests codegraph_query/node for recovery

Version Bumps

• Cargo.toml, npm/package.json, README.md → v0.2.4
