As a quick follow-up to Beta-2, this release fixes an issue with loading env vars, and adds some functionality and docs for the ctx.Ctx object.

Env var config values will now override code-provided values.
Ctx gains Set and Get methods to act as a pseudo K/V store.
Ctx gains a RequestID method to generate and store a request ID.
Ctx gains a Context method to get the underlying context.Context object.
