Ships PR #90's discovery/install split: `sim plugin catalog` for
discovery, `sim plugin install` accepts only explicit pip-installable
sources, custom JSON resolver and TTL cache removed.

Catalogue side migrated to schema_version 2 in
svd-ai-lab/sim-plugin-index (already live on main).
