📋 What's Changed

Fixed

• sshx run --json mirrors the structured reason for safety-policy blocks to
  stderr and reports the block as phase=admission, matching command mode.
• sshx run --script-file and --script-stdin warn when nested sudo is
  present without --sudo, explaining that the script occupies stdin and how
  to run the interpreter privileged.
• sshx sql database-name validation errors state the allowed characters and
  identify bracketed uppercase values that look like redacted placeholders.
