fix pH pyru issue on cpython

pypy will happily parse an unterminated python string whereas cpython
will not, this cuased the return value "'pH" to break pyru, however
the underlying issue was that the valued returned by the parse for pH
followed an old convention and should instead return ('quote', 'pH')
as it does now

fix iso8601duration and postnatal-day parsing conflicts

the underlying issue was that AT_MOST_ONE will succeed on the empty
string, also added the AND combinator to make it simpler to express
cases where multiple parsers are incomplete when run alone but ensure
correctness when run together and allow far more compact expression
that e.g. writing out all the order dependent parser rules by hand

the postnatal-day parser will match the prefix of malformed durations,
but that is ok because there will be a non empty rest which can be
used to detect the failure case

use quote internally, and implement namespaces

('quote', <expr>) is much easier to work with internally than "'expr" because
it more consistent and explicit rather than having implicit knowledge of
where we expect quotes by convention

implemented namespaces for that protc: and param: don't fight eachother
in the python impl (sigh python naming conventions), despite the fact
that in racket it doesn't matter at all
