First installable release since 0.10.0. 0.11.0 was tagged but never
published to npm, so this release contains both: the record-identity
work described under 0.11.0, and the vocabulary and validator work
described under 0.12.0.

Records that previously validated against nothing are now actually
validated. Five health record classes the CLI had been emitting without
any definition behind them are defined and shaped; cascade validate
reports how many subjects it actually checked rather than returning PASS
over zero constraints; and the shape-sync script, which had never copied
health.ttl at all, now drives every bundled file from one list.

On the 19-file reference pod: 278 of 448 subjects checked, up from 156,
and zero Cascade-typed subjects match no shape, down from 122.

A validation regression after upgrading means the validator improved,
not that the data degraded. See the 0.12.0 changelog section, including
the known date-datatype finding on C-CDA imports, which is deliberately
not patched in this release.
