A fix for model removal. hedos rm reported a model deleted but hedos ls kept showing it.

• Removing a model now drops its record from the shelf, not just its files. rm deleted the weights (or the Ollama tag) but never removed the record from the registry that ls reads, so the model lingered — and a rescan only re-flagged it as missing rather than dropping it. A removed model now leaves the shelf immediately and stays gone.
