Renames the setup bundle from OgmiosSetup.app to Ogmios.app so macOS permission prompts and System Settings show just 'Ogmios'. OgmiosRunner.app (the persistent XPC helper) unchanged.

• Codesign identifier is now reverse-DNS (org.ogmios.setup, org.ogmios.runner, org.ogmios.xpc.client) to clear stale macOS TCC cache entries that caused accessibility prompts to show old brand names.
• NSApp is re-activated between wizard alerts so the setup window stays foregrounded after macOS permission dialogs steal focus.
