feat: add MIDI note output for trigger channels

Send trigger 1/2/3 as MIDI Note On/Off messages with configurable
channel, destination, note numbers, velocity, and accent velocity.
Defaults follow General MIDI drum map (C2/D2/F#2). MIDI is off by
default; CV outputs continue to work independently.
