Fixed

• Runtime-created Consultant NEEDS_PLANNING handoff incidents now inherit
  root lineage from their source work item. Under an open closure target, the
  generated planning incident remains same-lineage claimable instead of being
  filtered out while the source task sits blocked.
• Incident markdown now accepts Status-Hint: incoming|active|blocked|resolved,
  so Consultant-authored diagnostic incidents with explicit incident state are
  not quarantined solely because they carry a status hint.

  used for the generated planning incident.
