release: v3.13.4 — middleware + headers parity (tina4-book#141)

Parity with tina4-python 3.13.4. Three bug fixes:

PY-10-02 (SECURITY): middleware() no longer mutates noAuth. Write
routes stay Bearer-token-gated when custom middleware is added.
Use ->noAuth() to open explicitly.

PY-10-03: $request->headers now uses Tina4\CaseInsensitiveArray
(ArrayAccess + IteratorAggregate + Countable). Mixed-case lookups
return the value.

PY-10-01: function-style closures with 3 args (req, resp, next)
form an Express-style continuation chain wrapping the route
handler. Class-based before*/after* dispatch unchanged.

2,858 tests passing.
